Tightly coupled systems where one small fault cascades and takes down the whole machine.

The parts are individually sound and the couplings are not. A single fault propagates faster than anyone can interrupt it, and the size of the failure has almost nothing to do with the size of the fault that started it.

How to spot it. Look at what cannot be switched off independently. That is where the cascade will run.
